  • Positive By Design LLC - Relationshiphip Intensives

Intensive Relationship Counseling A Marriage or Relationship Intensive Weekend is a concentrated version of Intimacy Therapy designed to be a private, dedicated weekend working with one of our associates over three days. We also provide small retreats for up to four couples hosted at various hotels, resorts, or bed and breakfast locations. Should we consider a Marriage/Relationship Intensive? We (are): Unable to attend traditional sessions on a weekly basis due to scheduling conflicts, location, or other family commitments Desire quick results for the distress in our relationship, or we simply desire to strengthen our connection Are influential in our community or live in a small town/ neighborhood and desire more privacy while working on our relationship Able to invest in the relationship without using insurance (insurance does not cover this type of service) During the three day intensive, you will have professional guidance to explore concepts of intimacy (in-to-me-see), including self, conflict/communication, affection, and spiritual intimacy if desired. You will have dedicated and focused time to learn how each of these components help you develop the long term love you have desired. This type of work normally takes 10-12 weekly sessions to achieve results. Intensive Format (*can be individualized to meet your needs) The Intensive weekend normally starts on Thursdays, and last through midday on Saturday. We meet for a total of 12 hours during the three days together. Many couples choose to spend extra time either before or after the weekend to both prepare and enjoy time away from home relaxing and connecting more deeply. Day One (approximately 4.5-5 hrs.) 9:00-9:45am: Couples' Session We will spend time exploring your vision for long term love, and the current concerns in the relationship. 9:45-10:30am: Individual Session A (Self-intimacy) This will be time to explore individual needs, wants, or desires without partner pressure. *There is a Starbucks, Panera, McDonald's...within 2-5 min. of my office. You may also relax with a journal, book, or to music in my waiting area. 10:30-11:15am: Individual Session B (Self- intimacy ) Partner A now gets a short break, as Partner B explores needs, wants, or desires without partner pressure 11:15-12 noon: Couples' Session We will reconvene together to overview the process for the rest of the weekend, including how we might explore each aspect of Intimacy Therapy. This will be a time to further commit to the process of deeper connection by acknowledging the negative patterns of thoughts and behaviors that have developed. Lunch/Afternoon Break This is your time to have a nice lunch, take a walk in a park, to nap or meditate-refuel for the evening,...I will provide you with a list of several local restaurants close to my office or your chosen hotel as needed. I will also include a list of local parks or scenic areas to just relax. There are several venues within 5-10 min. of my office. 4:30-6:00pm: Couples' Session (Conflict/Communication-Intimacy) We will begin to apply principles of positive and proactive communication to promote growth in the relationship, while sharing difficult emotions and thoughts. Evening Recommendations I encourage a nice dinner, and I will forward some restaurant ideas based on your preferences. This is an important time for you to both relax and enjoy each other, as well as time to talk and reflect on your day. I encourage you to plan for adequate rest and recovery, and you may want to make reservations for Friday lunch or dinner, if you have not already. Day Two (approximately 4.5-5 hrs.) Friday Morning Consider starting your morning with light exercise, mindfulness exercises, and a nice breakfast at your hotel or local spot. 9:00am-12:00pm: Couples' Session (Conflict/Communication-intimacy) We will continue to explore how to increase intimacy through positive communication -focusing on areas of tension and differentiation that you tend to avoid. There will be breaks as needed. Lunch/Afternoon Break This is your time again for a nice lunch, or even a quick nap. You could also opt for a couples' massage - I can provide a recommendation for a few spas within 2-10min. from my office. I will include information in your initial packet to schedule that ahead of time if desired. The venue closest to my office is Massage Envy or Massage Luxe, and I can typically arrange availability for two massage therapist on Thursday or Friday afternoons. 4:30-6:00pm: Couples' Session (Affection-intimacy) This 90 min. session will be used to explore how well you give and receive affection. This will be a session about more clearly showing understanding and supporting each other's "love language." We will examine both how well you give and receive affection. Evening Recommendations This would be a great time to explore an area of St. Louis you haven't experienced. I will likely give you homework that focuses on how to show love and care for your partner. *Don't stay up too late though; we still want you prepared for your final day. Day Three (approximately 2.5-3 hrs.) 9:00-12:00pm: Couples' Session (Affection-intimacy + Spiritual- intimacy if requested) The final session focuses on the momentum gained in the first two days. We work together on clearly defining how to sustain long-term love. If requested, we can also plan for how you engage in a faith walk together, or more broadly how to function as truly intimate partners (A WE TEAM). The latter part of this session will include a symbolic gesture that honors our work together and your commitment to follow through with a plan for continued growth as you both move forward in love. If desired, we can also discuss the possibility of a tune-up session anytime after your departure (including online conferencing for clients outside of the St. Louis Metro area). Whether local to St. Louis, or from out of state, you may choose to continue with additional sessions to deepen our work together. Call Now to Schedule Your Free 15-30 min. Consultation

  • Positive Design Coaching | positivebydesign

Positive Design Coaching Positive Design Coaching is the general term we use for many different types of coaching available from our associates. *It is important to note that coaching is not psychotherapy; there are no clinical diagnoses, goals, or interventions. The International Coaching Federation (ICF) defines coaching as partnering with clients in a thought-provoking and creative process that inspires them to maximize their personal and professional potential. Available: http://www.coachfederation.org/need/ Positive Design Coaching then is an ongoing professional relationship that helps produce extraordinary results in clients' lives, careers, businesses or organizations. Through the process of coaching, clients deepen their learning, improve their performance, and enhance their quality of life. We have a broad range of expertise and experiences in coaching areas such as Life, Business/Executive Leadership, Relationships, Health & Wellness, Sport (see Champion By Design ), among other areas. We can match you with our best suited associate to support your present needs. Choosing Your Best Path! Design thinking in behavioral health and well-being coaching supports strategic, intentional research and positioning for creating a new, more positive habit of thinking, doing, and being. In this way, design refers to a growth process where an individual or organization can explore possibilities with curiosity, logic, system dynamics, and innovation in mind. This has the net affect of promoting optimal results, including productivity and more happiness, by engaging in authentic, purposeful tasks and activities. Backwards Design is how we help you choose the best path towards achieving your goals. Stephen Covey (1989) says, To begin with the end in mind means to start with a clear understanding of your destination. It means to better understand where you're going so that you better understand where you are now so that the steps you take are in the right direction. The Seven Habits of Highly Effective People Balancing Daily! "Through balancing is where happiness and joy lives!" - David M. Harris, Founder Balancing is an active process that never stops. There is no absolute stage or point when anyone reaches a perfect equilibrium, combining both control over self and what life throws at us. The key is devleoping strategies and practices that support self-fullness (filling up your internal balancing reserves, to be at your best, allowing authenticity and purpose to overflow externally). This promotes a system of fullness because others are more likely to reciprocate their best to you from this position. The old cliche is appropriate, "You cannot care for others if you do not care for yourself." Being a Star in Your Life! Once you have developed a positive design for growth, in any area of life, leadership, performance, or business,...and your are mindful of preserving self-care and your balancing reserves, all you have to do is live it forward. Be courageous in daily living; dare to be great; avoid being held prisoner by a fear that past habits or mistakes will somehow come true again. You Are Positive By Design! Live in such a way that even if someone speaks negatively about you, no one would believe it!

  • FEES & PAYMENT | positivebydesign

Fees & Payment - You r care is worth th e investment, but be informed when deciding to seek service. How is therapy or coaching paid for? Generally speaking there are three ways to pay for service: Fee for Service or "out of pocket/private pay” : The client pays the therapist or coach directly. This allows you to have control over who you see, the type and goals of therapy or coaching, how long services will last, and who has access to your private information. *Note that private pay is the only way to pay for coaching. Insurance only covers medically necessary treatment (i.e. - you must have a diagnosis to substantiate treatment). In-Network Insurance or “Third party payment” : You and your insurance company each pay for a portion of the service. Your portion is known as a deductible, and/or co-pay/coinsurance. To use third party payment, the therapist must have an agreement or be In-network with the insurance company (see info below) . Third party payment gives the health insurance company some control over which therapist you see, the amount of money or time allowed for the therapy, and the type of therapy utilized. This may affect the goals of therapy, and for billing purposes, your confidential information. Once confidential information is sent to the insurance company, we cannot guarantee its privacy. Out-of-Network Payment : In some situations, when a therapist is not in your insurance company’s network, you can pay for the services directly, and then you can file to receive out-of-network benefits that are paid directly to you. (see the following for more information) In each case, you are responsible for paying your portion for services the day of your appointment. You can discuss the advantages and disadvantages of each payment type with your therapist/coach and come to an informed decision that is best for you. When cost is an issue, you may need to use your health benefits to access services. We encourage you to call for more information if you are currently experiencing financial hardship. We never want someone to not seek the care they need because of financial limitations; we may be able to help and/or refer you to someone who can. Cost and Payment Policy Methods of Payment: Cash, Check, Credit Card (MC, Visa, Discover American Express), Payment is due at the time of your session Rates: For services not covered by insurance or EAP, we charge a flat rate of $135 for a standard session. (*subject to change by each independent provider's policy) $135.00 for 60 minute initial intake session $200.00 for extended initial intake session (75-90 min.) $ 75.00 per 30 minute therapy session $135.00 per 38 – 52* minute therapy session (standard 45 min.) $135.00 per 53 – 60* minute therapy session (standard 60 min.) $200.00 per 75 – 90 minute therapy session (currently, this is only an option for those who are paying full fee without insurance) *Why do we charge the same rate for 45 vs. 60 min. sessions? Frankly, these are limits developed by the American Medical Association (AMA) in consultation with the American Psychiatric Association (APA). Mental health providers billing insurance must follow the limits for the sake of maintaining universally recognized standards for face to face time. Furthermore, many insurance companies do not reimburse the corresponding codes equitably. Some insurance companies will not even allow a 53-60 min. session without additional private information being shared to justify need. We prefer the longer session for clinical efficacy and effectiveness. We believe a different pay structure (especially as it affects our private pay clients) is unfair to clients, and for clinicians fair reimbursement alike. Services are provided and billed by or on behalf of each independent provider. Services may be covered in full or part by your EAP or health insurance plan if accepted by your provider (see questions to ask above). **Please be aware that with any health plans, there may be deductibles and copayments for which you would be responsible (review your policy or call your company for more information). Health Plans *Please note that each provider may not accept each insurance plan) Aetna Anthem Blue Cross and Blue Shield Cigna/Evernorth (Cigna Healthcare) Healthlink Tricare West United Behavioral Health (UHC/OptumHealth) Carelon Behavioral Health *If your plan is not listed or the provider you wish to see is out-of-network, we can provide you with documentation that you can submit to your insurance company to request reimbursements (reimbursement for out-of-network benefits) . Questions to Ask Your Insurance Company *You may be able to find information by clicking your company link in the column above left (Health Plans ). Does my plan cover mental health sessions? Does my plan cover only individual psychotherapy or will it also cover family or couples counseling? How many sessions does my plan cover in a year? How many sessions do I have left? Does my plan cover services to out-of-network mental health providers? What is the deductible I have to meet before coverage to an out-of-network provider kicks in? What is my copay or what percentage of treatment do I pay when seeing an out-of-network mental health provider? Is there a maximum amount per session the insurance will cover for an out-of-network provider? How much time do I have to file a claim for out-of-network services? Do I need pre-authorization or a referral from my PCP to see a therapist? If I need pre-authorization, do I need to call or does my therapist? What is the process to get reimbursed for out-of-network services? Health Plans Should I Use My Insurance Benefits? Be well informed when deciding to use your health plan to cover therapy. The unfortunate reality is that seeking mental health care through your insurance can sometimes have unplanned consequences. Insurance companies only cover care that is "medically necessary". This means, that they will typically only cover therapy for issues that have a recognized mental health diagnosis attached to them. Your provider will be required to submit a diagnosis in order to get reimbursed for any therapy that you engage in. When submitting a claim to your health insurance, you permit your provider to provide the clinical information that the insurance company requires to substantiate the medical necessity of your care. Thus, your diagnosis and the supporting evidence for that diagnosis becomes part of your health record. Some feel this is a risk that could potentially impact other areas of life that take your health record into account. Please be sure to talk with your therapist about any concerns before moving forward with sessions. By choosing to not use insurance, you have more choice about who you see, how long you stay in therapy and what your therapy looks like. You also gain a higher level of privacy related to your health information. When cost is an issue, you may need to use your health benefits to access services. Using health insurance may be the only way for you to get treatment. Ultimately, you can look at the advantages and disadvantages of each option and make an informed decision about what will be best for you.

  • Positive By Design LLC - What is Positive Design?

What is Positive Design? “Life is best designed by planning backwards, with the end in mind. We then can experience happiness and joy through living the plan forward, with authenticity, intimacy, mental resilience and agility - balancing through life changes and innovating for growth " - David M. Harris, founder We have adopted Positive Design (a blend of Positive Psychology and Design Thinking) as a model for thinking about and implementing dynamic, informed plans to create a more positive path in life. This is not to say that life can just be planned; however, your life can have guiding principles and intentional moves towards positive habits of well-being. Positive Design focuses on understanding your story and unique concerns (empathy), contributing to testing new ideas and adaptations in life. This generative process is the core of a human-centered approach for living a positive, productive life. Thus , Positive Design influences overall well-being as you create meaningful experiences, increase positive emotions, and pursue connections with people that enhance a sense of purpose and belonging. Following are summaries of the theoretical underpinnings of Positive Design. Positive Psychology Often credited as the "Father" of Positive Psychology, Dr. Martin Seligman, University of Pennsylvania, says there are five measurable elements that contribute to well-being, human flourishing (P.E.R.M.A.):see video below (available: https://www.authentichappiness.sas.upenn.edu/learn/wellbeing) Positive emotions - how to feel good Engagement - how to be fully absorbed in activities (flow state) Relationships - how to be authentically connected to others Meaning - how to lead a purposeful existence Accomplishment - how to achieve Design Thinking Adapted from the dSchool work at Stanford University, Design Thinking, as a process, focuses on understanding client need as the core of defining, developing and implementing solutions. It consists of five parts: Empathize : Work to fully understand the experience of the client for whom you are supporting the design process. Do this through observation, interaction, and immersing yourself in their experiences/story. Define: Process and synthesize the findings from your empathy work in order to form a client-centered view that will guide your design collaboration. Ideate: Explore a wide variety of possible solutions through generating a diverse set, allowing you to step beyond the obvious and explore a range of ideas. Prototype: Transform the ideas into a concrete plan of action, learning and developing more empathy as you explore potential outcomes. Test: Use observations and feedback to refine prototype ideas, learn more about the clients' adaptation(s), and refine the original client-centered view. This concept is displayed below, coupled with an overarching process (divergence-convergence). This is a cyclical process that is repeated as often as necessary to achieve desired results. Divergence : the phase where collaboration includes asking the right questions. This is an unpacking, insight-based process that is generative. Emergence : the "Grow Zone" where diverse ideas cause a stretch for all involved in the design process. You often must stretch your bias to find new solutions. Convergence : the action phase, a more focused implementation with desired results in mind. Figure 1 - A Framework for Positive Design *Figure 1 - A Framework for Positive Design is an adapted representation of another way that we conceptualize designing and planning for human flourishing. This framework, along with a backwards design planning sheet (see more about design thinking and backwards design under Positive Design Coaching ) is used to help you create a success map - connected to your unique vision for the life you love. (*adapted from Desmet, P., & Pohlmeyer, A. 2013 Nov 27. Positive Design: An Introduction to Design for Subjective Well-Being. International Journal of Design [Online] 7:3. Available: http://www.ijdesign.org/index.php/IJDesign/article/view/1666/595#2 We have also adopted the following manifest to represent our belief in the role of positive design as it relates to any form of human development that supports individual and collective flourishing (overall well-being). Positive Design Manifest (available: http://studiolab.ide.tudelft.nl/diopd/about-us/mission/ ) Positive Design…. (1) creates possibilities Positive Design envisions and realizes optimistic futures. Rather than merely reducing people’s problems, it offers them opportunities to improve their wellbeing. (2) supports human flourishing Positive Design uplifts people. It enables and inspires people to develop their talents, to increase their freedom, to deepen their relationships, and to contribute to their communities. (3) enables meaningful activities. Positive Design encourages people to balance pleasure and virtue. It stimulates people to engage in meaningful activities that are rooted in their deeply held values. (4) embraces rich experiences. Positive Design affects the complete pallet of human experiences. Beyond short-term pleasures, it focuses on lasting experiences that involve both positive and negative emotions. (5) accepts responsibility Positive Design is genuine in its purpose and intention. It takes responsibility for its short- and long-term impact on individuals as well as on communities and society.

  • No Surprises Act | positivebydesign

effective 1/1/22 Your Rights and Protections Against Surprise Medical Bills When you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing What is “balance billing” (sometimes called “surprise billing”)? When you see a doctor or other health care provider, you may owe certain out-of-pocket costs, such as a copayment, coinsurance, and/or a deductible. You may have other costs or have to pay the entire bill if you see a provider or visit a health care facility that isn’t in your health plan’s network. “Out-of-network” describes providers and facilities that haven’t signed a contract with your health plan. Out-of-network providers may be permitted to bill you for the difference between what your plan agreed to pay and the full amount charged for a service. This is called “balance billing.” This amount is likely more than in-network costs for the same service and might not count toward your annual out-of-pocket limit. “Surprise billing” is an unexpected balance bill. This can happen when you can’t control who is involved in your care—like when you have an emergency or when you schedule a visit at an in network facility but are unexpectedly treated by an out-of-network provider. When balance billing isn’t allowed, you also have the following protections: • You are only responsible for paying your share of the cost (like the copayments, coinsurance, and deductibles that you would pay if the provider or facility was in-network). Your health plan will pay out-of-network providers and facilities directly. • Your health plan generally must: Cover emergency services without requiring you to get approval for services in advance (prior authorization). Cover emergency services by out-of-network providers. Base what you owe the provider or facility (cost-sharing) on what it would pay an in-network provider or facility and show that amount in your explanation of benefits. Count any amount you pay for emergency services or out-of-network services toward your deductible and out-of-pocket limit. You are protected from balance billing for: Emergency services: If you have an emergency medical condition and get emergency services from an out-of-network provider or facility, the most the provider or facility may bill you is your plan’s in-network cost-sharing amount (such as copayments and coinsurance). You can’t be balance billed for these emergency services. This includes services you may get after you’re in stable condition, unless you give written consent and give up your protections not to be balanced billed for these post-stabilization services. Certain services at an in-network hospital or ambulatory surgical center: When you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers may bill you is your plan’s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can’t balance bill you and may not ask you to give up your protections not to be balance billed. If you get other services at these in-network facilities, out-of-network providers can’t balance bill you, unless you give written consent and give up your protections. You’re never required to give up your protections from balance billing. You also aren’t required to get care out-of-network. You can choose a provider or facility in your plan’s network. Additionally, Missouri law requires that patients pay only their in-network cost sharing amounts. These protections apply to any patient covered by a state regulated insurance plan but does not apply to a liability insurance policy, workers’ compensation insurance policy, or medical payments insurance issued as a supplement to a liability policy. If you believe you’ve been wrongly billed... Good Faith Estimate You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment and hospital fees. Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
